No need to make an appointment for a
"Life Certificate"
The consulate does not provide any form except the "life certificate" form. You must bring the documents you are required to sign.
You must present an ID card. It is advisable to present an Israeli passport
Each person who signs has to be proficient with the language of the document, otherwise translation is needed.
There are no translation services at the consulate, a list of translators is available on the website.
