    Marital status update/ Selection of name
    In order to update your marital status, please provide us with the following documentation:
    1.      Please fill out the form for a change of personal status.
    Need to be signed by both spouses.
    2.    Please fill out the form for a selection of name (even if you haven't changed your name).
    The forms needs to be signed by both spouses (3 signatures in total).
    3.   An original marriage/ divorce certificate sealed with an *Apostille (requests without apostille seal will not be accepted)
    4.    In case of a marriage, please provide a photocopy of your spouse's passport/ID 
    5.    Both people need to arrive to the consulate in person.
    6.   For payment methods, please see the "Payment Methods" page. (Paid to: “Consulate General of Israel”). No AMEX. No personal checks.​​​

    For the registration in the population registry (newborn abroad):


    1. Original birth certificate with an *Apostille seal (requests without apostille seal will not be accepted)​
    2. Certified confirmation of the birth from the hospital - In addition to the birth certificate. Examples for welcomed documents:
    - Document from the hospital stating the childbirth
    - Hospitalization or release form from the hospital
    - Proof of pregnancy tracking from the third trimester
    - Confirmation from the Gynecologist that treated the mother until childbirth, including the name of the mother, date of birth of the newborn, sex of the newborn, and name of the hospital where the birth took place.
    1. Registration for a new Israeli citizen born abroad.
    2. Photocopy of both parent's passports (Israeli passport for the Israeli parent)
    3. 2 passport photos
    ** The Israeli parent must arrive to the consulate in person with the child

     If you are willing to issue a passport for your child, please get the information from "issuing first passport" in the passports section.
     
     
    To order a certificate please provide the following:
    1.       For birth certificate, registration abstract or death certificate, please click here and fill out this form.
    Only birth certificates can be ordered in English.
    In Case of a birth certificate in English, please fill out the application in English.
    2.       For Certificate Attesting to Israeli Citizenship or Non-Citizenship, please fill out and sign this form.​
    4.       For information on a traveler (entrances and exits to/ from Israel), please click here.
    4.       For Request for a criminal/ non-criminal police recordplease click here. ​- The record is being ordered from Israel. Once it arrives at the Consulate, it can only be sent to the third party, who required this. Please provide the full address of the recipient and any further details if necessary (case number if necessary)
    5.     The consulate does not provide translations (besides birth certificate if ordered in advance)
    6.       Photocopy of an ID (Israeli passport for the Israeli citizens) for all the above
    7.       If you need an Apostille Seal, please provide $9 by credit card (download the form here) or money order (Paid to: “Consulate General of Israel”) for all the above.

    9.     Please add $12 (for FedEx Overnight Shipping) by credit card.

     Change a name
    In order to require for a change of name (first or last name), please provide the following documentation:
    1.       Please fill out the form for a change of name.
    2.      For the fee table please click here​ + $12 (overnight shipping)  by credit card (download the form here) or by one money order (Paid to: “Consulate General of Israel”). No AMEX. No personal Check.

    Military deferment (starts only at the age of 16.4)
    In order to settle your military deferment, please provide the following documents:
    1. The applicant needs to fill out the following 2 forms and sign (2 pages in total)​
    2. Please attach copies of both parents and the child's passports (Israeli passport for the Israeli parent)
     
    If you are unable to come in person to the consulate, the applicants' signatures need to be notarized by a notary public (download form) and also attach a photocopy of the applicant’s passport.
     
    If the applicant is a minor and need to extent\renewal the passport please look at chapter “passport extension/ renewal” for minors in the passport section.

    * The process usually takes up to a month.
     Once you have submitted your application, you must contact us after 3 weeks and check the status.

     

     Waiving of an Israeli citizenship

    The process of renouncing Israeli citizenship is intended for an Israeli citizen living abroad who wishes to renounce his Israeli citizenship for any reason (only if stayed at least 3 years abroad in the past 5 years). This is implemented by giving a declaration of the desire to renounce the citizenship, along with relevant documents and an application to the Ministry of the Interior for its authorization.
    It is important to note that the decision to authorize a renunciation of citizenship is made at the sole discretion of the Minister of the Interior or anyone appointed by him, and as long as such authorization is not given, the person who made the declaration is still an Israeli citizen. It should be noted that there have been cases in which the minister saw fit, for various reasons, not to authorize a renunciation of citizenship.
    A person who wishes to submit an application to renounce Israeli citizenship must report in person to the Consulate and implement the following:
    Please note:
    1. It is very important to fill out all sections of the form; otherwise the Ministry of the Interior will not handle the application.
    2. The process of renouncing citizenship cannot be implemented by mail. The presence of the applicant is mandatory!
    3. The process takes up to a year.
    The signatory undertakes, before any use of his Israeli passport, to check with the Consulate whether his Israeli citizenship has been cancelled
