Any applicant who was born in Israel can apply for himself / herself or for a first-degree relative only.
Death certificate
Every applicant for a family member who died in Israel.
· For Death certificate, if the applicant for the certificate is an interested party (such as an heir / lawyer of the interested party), a power of attorney from an attorney or an order from a court must be attached.
What is important to emphasize when filling out an application?
· Include the identity number of the person for whom the application is submitted
· If the application is submitted for a person who is not an Israeli citizen, a foreign passport number must be stated.

· Birth certificate can be in Hebrew or bilingual (Hebrew and English). The relevant form must be filled in according to the language of the requested certificate.
· Death certificate can be in Hebrew only.
