    Birth Certificate / Death Certificate 
    (during COVID-19 this service is provided by mail only. Please obtain the required documents and mail them to the consulate to apply).
     
    Who can request this certificate and for whom?
     Birth Certificate
    Any applicant who was born in Israel can apply for himself / herself or for a first-degree relative only.
     Death certificate
    Every applicant for a family member who died in Israel.
     ·         For Death certificate, if the applicant for the certificate is an interested party (such as an heir / lawyer of the interested party), a power of attorney from an attorney or an order from a court must be attached.
     
    What is important to emphasize when filling out an application?
    ·         Include the identity number of the person for whom the application is submitted
    ·         If the application is submitted for a person who is not an Israeli citizen, a foreign passport number must be stated.
     
    What language is available for this certificate?
    ·         Birth certificate can be in Hebrew or bilingual (Hebrew and English). The relevant form must be filled in according to the language of the requested certificate.
    ·         Death certificate can be in Hebrew only.
     
    What is the cost of ordering the certificate?
    The certificate is free.
     
    Is there an additional cost to order additional copies?
    Ordering additional copies does not involve any cost.
     
    How long does it take to receive the certificate?
    The process of preparing and receiving the certificate takes up to two months.
     
    Apostille services
    The apostille stamp is made in Israel, after issuance of the certificate and before it is sent to the mission. An apostille stamp is required if the certificate is intended for parties outside of Israel (for example, an American company) - see the cost of an apostille stamp on the “fees” page.
     
    What forms should I attach to the application?

    Who can request this certificate and for whom?
    Any citizen for himself or herself or for a first-degree relative only.
    ·         If the application is requested for a first-degree relative, a signed power of attorney must be attached with a notary public and a photocopy of the identity card or Israeli passport of the family member for whom the application is submitted
    ·         If the application is requested for a non-immediate relative, or for others, a power of attorney must be attached from an attorney or a an order from court
     
    What is important to emphasize when filling out an application?
    Include the identity number of the citizen or citizen for whom the application is submitted.
    ·         If the applicant is not the citizen for whom the application is submitted - an Israeli identity number or a foreign passport number of the applicant must be included
    ·         If an expanded prescription is required, this should be noted on the form

    Any person who has lived in Israel, whether as an Israeli citizen or a foreigner.

    Certificate of a Name Change / Name Selection

    Who can request this certificate and for whom?
    ·         A person who changed his or her first or last name is entitled to request a certificate indicating a change of name for himself / herself or for his / her minor children
    ·         A person who changed the surname after his / her marriage or whose marriage has expired has the right to request documentation for himself / herself only
    ·         For another family member or for another person - a power of attorney from a lawyer must be attached to the application
     
    What is important to emphasize when filling out an application?
    The current and previous name must be spelled accurately and clearly.
     
    What language is available for this certificate?
    This certificate can be in Hebrew only.
     
    What is the cost of ordering the certificate?
    First certificate after a name change / selection is free.
     
    Is there a cost to order another certificate?

    Who can request this certificate and for whom?
    ·         Applicants who wish to receive this certificate for themselves
    ·         A certificate in the name of a deceased person may apply for:
    ·         Parent for his minor son
    ·         First degree relative (wife, husband, son, daughter)
    ·         A second degree relative, with no first-degree relatives (for example, a grandchild who requests a certificate in the name of his grandfather, when the son also dies)
    ·         Heir to a deceased person, according to a will
     
    What is important to emphasize when filling out an application?
    ·         If the date of obtaining the citizenship is required - please indicate this on the form
    ·         If a bilingual certificate is required, this should be indicated on the form
